The report of the Bristol Royal Infirmary Inquiry revealed that the UK health service was badly in need of reform (Department of Health (DoH), 2001). It stressed that the patient’s perspective ‘must be included in the policies, planning and delivery of services at every level’. This inquiry was one of the initiators of the Government’s determination to introduce clinical governance through which all elements of the NHS are now mandated to be accountable for the continuous monitoring and improvement of their services.
