CNS inflammatory vasculopathy with anti-myelin oligodendrocyte glycoprotein antibodies in COVID-19
We present the case of a 44 year old female patient with a CNS inflammatory condition secondary to COVID-19, characterized by marked perivascular enhancement on MRI, negative SARS-CoV-2 PCR in CSF, anti-myelin oligodendrocyte glycoprotein (MOG) antibodies in serum, and rapid clinical and radiological recovery following treatment with intravenous methylprednisolone (IVMP) and plasma exchange (PLEX). We hypothesise that this unusual clinical and radiological presentation was caused by a combination of an anti-MOG antibody response and endothelial dysfunction accompanying COVID-19. The rapid clinical and radiological response to PLEX and IVMP was striking.
