Below-knee amputation for peripheral ischaemia is preferable to above-knee ablation, whenever the former is feasible. 2 large studies have shown that in 'geriatric' patients (defined as those over 55) the results of above-knee surgery are poor, with only 10% and 9.9% respectively of patients so treated subsequently being fitted with a prosthesis. Pedersen states that the current problem of the geriatric amputee 'is not prosthetic components or training, but preservation of the knee joint'.
