Radionuclide assessment of right ventricular function : studies in health and pulmonary heart disease
The study of right ventricular function is hampered by the complex and variable anatomical configuration of the right ventricle. Radionuclide techniques have been applied widely to evaluate the function of the right ventricle but are subject to problems with methodology. This thesis describes the use of right heart krypton-81m equilibrium radionuclide ventriculography for the study of right ventricular function. The definition of optimal techniques for image acquisition and analysis using krypton-81m equilibrium ventriculography are evaluated and the application of the technique to the calculation of specific indices of right ventricular systolic and diastolic function described. In addition, krypton-81m equilibrium ventriculography is applied to the non-invasive estimation of right ventricular volumes. The resting RVEF and exercise response of the normal right ventricle is defined and compared with patients with chronic obstructive pulmonary disease. Krypton-81m equilibrium ventriculography is used to evaluate changes in right ventricular systolic and diastolic function following treatment with xamoterol, a beta-1 adrenoceptor partial agonist with inotropic properties, in patients with pulmonary heart disease. Changes in right ventricular haemodynamics in response to an increase in right ventricular afterload induced by hypoxia in normal subjects is assessed. Right heart krypton-81m equilibrium ventriculography is ideally suited to the non-invasive evaluation of right ventricular function. In addition to the measurement of RVEF, a useful index of global systolic function, specific indices of both systolic and diastolic function can be derived. The imaging technique can also be applied to the measurement of right ventricular volumes. This allows a detailed evaluation of right ventricular function in response to changes in loading conditions particularly that following therapeutic intervention.
