Assessment and recovery of cognitive behaviours and cognitive impairment after severe traumatic brain injury

This thesis addresses recovery of cognitive behaviours and cognitive impairments after severe traumatic head injury. The main aims were (i) to develop a scale to assess early cognitive behaviours after traumatic brain injury, (ii) to use this scale to identify a sequence of recovery of these behaviours and (iii) to identify differences in patterns of recovery. Three separate but related studies are presented. The first describes the development of the scale encompassing the earliest cognitive behaviours observed during the process of recovery from severe traumatic brain injury. The second investigates whether it is possible to influence the order of recovery and the third is a follow up of the first study cohort, describing outcome four years later and examining whether any of the behaviours on the scale are predictive of outcome.

A range of methodologies was used to address the aims. In the first study, observation was used to identify potential behaviours for inclusion in the scale. A single case methodology was employed in the second. The third study was a survey describing the cognitive status of the sample at 3-4 years after injury.

The main results of the first study were (i) development of a scale; (ii) identification of a sequence of recovery and (iii) identification of patterns of recovery. Results from the series of single cases showed that the sequence of recovery identified in the first study could be altered by intervention. In the third study, the relationship between time taken to achieve the early behaviours and test performance at follow up was explored and behaviours which were associated with scores at outcome identified.

The overall result of the research is a scale which is appropriate to assess patients from the stage of emerging from coma until they emerge from Post Traumatic Amnesia (PTA). The thesis concludes with a discussion of the overall work in relation to the background literature and the aims.
