An initial study was conducted to explore the interactional processes involved in everyday face-to-face interactions between informal carers and health care workers on a hospital ward caring for older people.  Informal carers’ naturally occurring interactions with health care workers at the ward desk were videotape recorded over a three-month period.  The data comprised 30 dyadic or group interactions.  Analysis was based upon the principles of the study of institutional interaction.  The findings from the initial study illustrated the interactional relevance of knowledge and competence to the organisation of informal carer-health care worker relationships on hospital wards.

A second empirical study aimed to explore how informal carer and health care worker relationships were organised and managed during structured collaborative care meetings in a rehabilitation unit caring for older people.  Ten initial collaborative care meetings attended by the patient, his or her formal carer(s) and health care professionals were videotape recorded.  Analysis again employed the principles of the study of institutional interaction.  The interrelationship of participants’ local negotiation of their agendas and the social structure of their relationship was analysed.  Participants’ discursive practices and related actions illustrated how informal carers and health care workers negotiated various institutional constraints and made relevant their shared or exclusive caregiving knowledge and interactional competencies.

It is argued that the research extends existing conceptualisations of the relationship between informal and formal carers by identifying the active role of informal carers in how the relationship is established.  By defining the relationship as a social action, an understanding has also been obtained of how informal carers manage their expertise as a negotiated interactional accomplished of everyday life.
