This paper was produced in response to a national invitation in July 2025 to submit proposals to The Single Patient Record (SPR) Development Team for NHS England, and to inform direct discussions with the Team in November 2025.

The software architecture of the locally developed UHS Lifelines Electronic Patient Record (EPR) interface at University Hospital Southampton NHS Foundation Trust suggests a simple, scalable model for the integration of the diverse information sources which comprise a citizen's personal health record in a unitary overview and navigation space.

This approach is anticipated to bring significant structural, operational and efficiency benefits for healthcare teams, and far better oversight of personal health records for every UK citizen
